Treatment time begins at the time of appointment, so I will plan to arrive at your home 30 minutes early to set up and provide a health intake form, discuss session goals, use the restroom if needed, etc.. 

Appointment will end on time if you arrive late (not early) without notice. 

Cost for the session will remain the same.

I am fully vaccinated against COVID-19, but am happy to wear a mask if requested.
If you are unvaccinated, I request that you wear a mask.

If you experience ANY symptoms of cold or flu (fever, cough, etc.), you must reschedule. 
 

​Upon booking, 50% of total cost will be required via Venmo or Zelle.
The remaining 50% is due at time of service.

Gratuity is appreciated.

Any cancellations or rescheduling should be made at least 4 hours in advance, but

accommodations may be made in the case of emergency.

Failure to cancel more than 1 hour prior to the session start time

will result in retention of 50% of booking deposit as well as full pre-payment for your next appointment.

Failure of the client to show up for an appointment with no notice will result in

retention of 100% of booking deposit as well as full pre-payment for future appointments.
If I fail to show up for an appointment, your deposit will be refunded and your next appointment will be 50% off.
 

Clients under the age of 18 are welcome with a parent or guardian present.

I do have a minor health intake / release form on request, 

to be filled out by the parent or guardian and the minor client. 

It is vital that you provide an accurate health intake, as there are certain conditions that could be made worse by massage. I retain the right to refuse service to anyone for whom I feel

massage would be unhealthy, damaging, or inappropriate (for you or me).

Please be sure to update your health records periodically.

I prefer not to engage in any discussions of mutual acquaintances or my personal life. 

Nothing that is discussed in the treatment room will be shared with anyone else – unless explicit permission has been given by you to discuss the treatment plan with a physician or chiropractor, or if required by law.

Conversations of a sexual nature are not appropriate and will be curbed.

Any sexual behavior, jokes, advances, references to inappropriate conduct, or requests for sexual acts will not be tolerated. The session will be terminated, full payment expected, and you will no longer be a client.
